Position Summary
The Client Care Coordination Specialist serves as a vital liaison between patients, caregivers, referral sources, and the clinical team. This role is responsible for coordinating patient intake, scheduling home health services, managing referrals, and ensuring exceptional customer service throughout the care process. The ideal candidate is organized, compassionate, detail-oriented, and experienced in healthcare administration and home health operations.
Key Responsibilities
Client Intake & Admissions
Process incoming patient referrals in a timely and professional manner
Verify patient eligibility, insurance coverage, and authorization requirements
Coordinate admissions, onboarding, and start-of-care scheduling
Manage patient intake workflow, data entry, and electronic documentation
Maintain accurate and confidential patient records in compliance with HIPAA regulations
Scheduling & Care Coordination
Coordinate caregiver and clinical staff schedules to ensure proper patient coverage
Manage patient admissions, transfers, rescheduling, and discharges
Communicate schedule changes promptly with patients, caregivers, and clinical staff
Monitor staffing coverage and assist with resolving scheduling conflicts
Support continuity of care through effective coordination and follow-up
Other assigned duties within the scope of services for client care coordination workflows
Customer Service & Patient Relations
Answer and manage incoming phone calls and inquiries professionally
Serve as a primary point of contact for patients, families, referral sources, and staff
Provide compassionate customer service and timely follow-up regarding patient needs
Address concerns and escalate clinical or operational issues appropriately
Build positive relationships with clients and community partners
Insurance & Documentation Support
Assist the Credential Specialist with Medicaid, Medicare, Private Pay, VA, and insurance documentation requirements
Support clinical staff with required intake and compliance documentation
Ensure timely completion and submission of forms, authorizations, and records
Maintain knowledge of payer guidelines and agency procedures
Referral Development & Community Relations
Assist in building and maintaining referral relationships with hospitals, physicians, rehabilitation centers, case managers, and community organizations
Support outreach efforts and follow-up communications with referral partners
Help maintain professional relationships that contribute to agency growth and patient satisfaction
Qualifications
High school diploma or equivalent required; Associate’s degree in Healthcare Administration or
related field preferred
Minimum of 2 years of experience in home health, healthcare scheduling, medical office administration, or care coordination preferred
Knowledge of Medicare, Medicaid, Private Pay, and insurance verification processes
Strong communication, customer service, and organizational skills
Ability to multitask and work in a fast-paced healthcare environment
Experience with WellSky, EMR/EHR systems, scheduling platforms, and Microsoft Office proficient
Understanding of HIPAA compliance and patient confidentiality standards
