We are seeking a compassionate and detail-oriented Client Care Coordinator to join our team. In this role, you’ll ensure high-quality care for our clients while training, supporting, and supervising caregivers. This is an excellent opportunity for an LPN, RN, or LCSW.

What You’ll Do
Lead comprehensive client care by conducting thorough assessments, developing and regularly reviewing personalized care plans, performing home visits, and serving as the primary point of contact for clients and families to ensure exceptional satisfaction and quality of care.
Oversee all aspects of caregiver management, including supervision, ongoing training, in-the-field coaching, caregiver performance feedback, and participation in recruitment and onboarding to maintain a high-performing care team.
Serve as a key liaison between clients, caregivers, leadership, and external partners — including healthcare providers and community organizations — ensuring seamless communication, comprehensive care coordination, and timely updates on client and caregiver status.
Maintain accurate client records, ensure regulatory compliance and confidentiality, communicate changes in client condition to leadership, and support operational functions including billing accuracy and after-hours on-call duties.
Commit to continuous professional growth by staying current on industry trends and best practices, contributing to a culture of excellence, accountability, and compassionate care delivery.
Requirements
Bachelor's degree required in social work, nursing, healthcare administration, or related field
Minimum of 3 years of experience in senior care or related healthcare settings
Ability to provide care to seniors
Strong knowledge of caregiving best practices and safety protocols
Excellent communication and leadership skills
Valid driver’s license and insured vehicle
