Position Summary
We are seeking a licensed Physical Therapist (PT) to join our interdisciplinary team. The PT will provide skilled therapy services to adult and geriatric patients in their homes, focusing on restoring function, improving mobility, and preventing physical disability.
Responsibilities
Perform comprehensive evaluations of patients’ functional status, including musculoskeletal, neurological, and cardiopulmonary assessments.
Develop individualized treatment plans and set measurable, achievable goals in collaboration with physicians and the care team.
Provide therapeutic exercises, gait training, balance activities, and patient/caregiver education to promote safety and independence.
Document assessments, progress notes, and care plans accurately and in a timely manner, adhering to state, federal, and agency standards.
Communicate effectively with patients, families, physicians, and other team members to coordinate care.
Supervise Physical Therapist Assistants (PTAs) and support staff when applicable.
Qualifications
Current Indiana Physical Therapist license.
Graduation from an accredited Physical Therapy program.
Current CPR certification.
Valid driver’s license, reliable transportation, and proof of auto insurance.
Strong clinical assessment and critical-thinking skills.
