In this role, you will play a critical part in the client care journey by conducting Comprehensive Needs Assessments (CNAs), completing intake documentation, and accurately populating client profiles within the Electronic Health Record (EHR). You will collaborate with guardians, service providers, and Managed Care Organizations (MCOs) to coordinate services, manage referrals, and facilitate service authorizations. As a key point of communication, you’ll provide timely support to internal teams and leadership, ensuring that all assessments, documentation, and follow-ups are completed with precision and care.
