    At CareTrack, we believe some of the most important care happens between office visits.

    Our Clinical Care Coordinators connect directly with patients…helping them stay engaged in their care plan, understand their health trends, and feel supported at home.

    This role primarily supports CareTrack's Remote Patient Monitoring (RPM) program , while occasionally assisting with other patient outreach and care coordination initiatives as needed.

    This is meaningful work.

    A Day in the Life_**

    You begin your day reviewing scheduled patient outreach…checking notes, recent vitals, and identifying opportunities to encourage healthier habits or address concerns.

    Your first call may be a patient with elevated blood pressure readings. You discuss trends, reinforce the provider's care plan, answer questions, and encourage continued monitoring.

    Later, you connect with a patient who's making progress…you celebrate wins, offer encouragement, and help keep momentum going.

    Throughout the day, you may:

    • Conduct monthly patient outreach calls
    • Review monitoring trends and discuss progress
    • Reinforce provider care plans
    • Help patients understand the importance of consistent readings
    • Encourage adherence to medications and healthy habits
    • Identify concerns that may need escalation
    • Coordinate with internal care teams and providers
    • Document conversations clearly and accurately
    • Occasionally support broader patient outreach initiatives as needed

    At the end of the day…patients feel cared for, supported, and more confident in their health journey.

    • Build trusted relationships with RPM patients
    • Conduct proactive outreach and meaningful follow-up
    • Encourage adherence to provider care plans
    • Review patient progress and outcomes
    • Identify changes or concerns requiring attention
    • Maintain accurate electronic documentation
    • Coordinate with providers and internal teams
    • Support patient education and engagement initiatives
    • Deliver compassionate, consistent patient experiences

    What We're Looking For

    • Medical Assistant (MA), CNA, LPN, or similar clinical background preferred
    • Strong communication and listening skills
    • Compassionate, patient-centered mindset
    • Comfortable discussing chronic conditions and care plans
    • Organized and detail-oriented
    • Comfortable with EHR/documentation systems
    • Ability to work independently while staying connected to a team

    Preferred experience:

    • Primary care / family medicine
    • Chronic disease management
    • Patient outreach / care coordination
    • RPM / CCM / AWV familiarity
