As a Care Manager, you will be a trusted guide for patients and families navigating complex health journeys. This role blends your clinical expertise with a holistic approach to care, addressing medical, behavioral, and social needs. You’ll work remotely, but your impact will be deeply personal, helping members achieve wellness and autonomy through education, support, and compassionate coordination
What You’ll Be Doing
Build meaningful relationships with patients and families, understanding their goals, barriers, and motivators.
Creating personalized care plans that go beyond diagnoses, addressing social determinants, emotional well-being, and behavioral health when applicable.
Coordinate care across providers and settings, ensuring safety, quality, and continuity.
Educate and empower members to make informed decisions and navigate the healthcare system confidently.
Identifying and coordinating access to community-based resources and services and ensuring implementation for the member.
Collaborate with an interdisciplinary team (inpatient, outpatient, home health, etc.), sharing insights and advocating for whole-person health.
Use data as a tool for insight—not the driver of care—to identify opportunities and measure impact while keeping the patient at the center.
Maintain compliance with regulatory standards while prioritizing compassion and connection.
Qualifications
Qualifications – What You’ll Need to Have
Master’s degree in social work (MSW)
Minimum of five (5) years of clinical experience across diverse healthcare settings including medical and behavioral health.
Strong background in care management as well as complex and chronic condition management, both inpatient and outpatient.
Skilled in motivational interviewing, patient education, and patient advocacy.
Ability to work independently in a remote environment while staying connected to the team.
Comfort with technology and multiple EMR systems.
Possession of software knowledge including word processing and spreadsheets, computer skills including MS Word, Excel, Access, PDF, Outlook, etc.
What We’d Love for You to Have
LCSW, LCSW, LCPC, or LCP
Perinatal Mental Health Certification (PHM-C)
Specialty Certification in Care Management (CCM or ACM)
Previous remote and/or telephonic work experience
Knowledge of utilization review, quality improvement, managed care, and/or community health

COMPENSATION:
The listed compensation range listed is paid bi-weekly per our standard payroll practices. Final base pay decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.

REMOTE WORK TECHNICAL REQUIREMENTS
Guidehealth is a fully remote company. We provide new employees with the necessary equipment to function in their role at no charge to the employee. Employees provide their own internet connection, capable of conducting video calls on camera and connecting to various internal and external systems.
