Join a Team That Cares – and Supports Your Work-Life Balance!
At Boost Home healthcare - North Shore, we believe in delivering exceptional care to our patients and fostering a positive, flexible, and balanced work environment for our team.
We’re looking for a Licensed Practical Nurse (LPN) to serve as our Clinical Coordinator to join our growing team and become a key part of our healthy, respectful, and fun team culture. As a member of our team, you will be playing a central role in referral intake, care coordination, and office operations — with the opportunity to provide occasional patient support in the field when needed.
We take work-life balance seriously and are building an employee-centric culture while supporting our mission to make it easier for patients to focus on recovery and wellness by personalizing and coordinating care. We're thoughtfully designing our teams to serve patients on the North Shore and Merrimack Valley that are within a reasonable range from home.
What You'll Be Doing
Clinical Intake & Care Coordination
Coordinate intake of new referrals, including reviewing clinical information and ensuring appropriateness for home health services
Communicate with referral sources, patients, and families to facilitate smooth admissions
Verify patient insurance eligibility and obtain necessary authorizations to ensure smooth reimbursement processes
Collaborate with the RN Clinical Manager to ensure timely starts of care: coordinate patient visits and align care needs with staff availability
Agency, Office & Administrative Operations Coordination
Support daily office operations related to scheduling, intake, orders and documentation
Serve as a central point of contact between clinicians, patients, referral partners, and payers
Support Compliance: Maintain accurate records in the electronic medical record (EMR) and stay on top of ever-changing Medicare, Medicaid, state regulations, and agency requirements
Assist with onboarding coordination, payroll support, and basic administrative workflows as needed
Provide on-call support on a rotational or as needed basis
Clinical Support (As Needed)
Provide intermittent skilled nursing support in the field under the direction of the RN, based on agency needs
Implement nursing care plans in accordance with physician orders and agency policy
Observe and report patient status changes to the Clinical Manager, RN and/or physician
Educate patients and families on care needs and treatment plans
What We're Looking For
Education: Graduate of an approved school of practical (vocational) nursing
Active LPN license in the state of Massachusetts
Experience: Minimum 1 year of home health experience preferred
Current BLS certification
Tech-Savvy: Comfortable using Microsoft Office and electronic health records (EHR); experience with systems like KanTime, Axxess, WellSky, or HCHB is a plus.
Who You Are
Clinically grounded and administratively sharp
Calm under pressure and great with people
Motivated by building something meaningful
Comfortable wearing multiple hats and working in a fast-paced, early-stage environment
Strong communicator and team motivator
Positive, solution-oriented mindset with strong attention to detail
