What You’ll Do • Conduct client assessments and develop thoughtful, personalized care plans • Provide clinical oversight across private duty home care and four RCFE (board & care) homes • Support and guide caregivers through training, mentoring, and ongoing development • Foster a culture of consistency, compassion, and accountability within the care team • Collaborate closely with families and referral partners to ensure a supportive, high-quality experience What Makes This Role SpecialMeaningful Relationships: Build lasting connections with clients, families, and your team • Supportive Leadership: Lead in an environment that values empathy, trust, and collaboration • Steady Impact: Shape care experiences in a way that truly improves daily life for others What You Bring • Active California RN license in good standing, and without restrictions • 10+ years of experience in hospital, skilled nursing, home health, and/or hospice settings • Strong clinical judgment paired with a calm, thoughtful approach • A natural ability to guide, support, and develop others • High emotional intelligence—you connect easily while maintaining professional boundaries • A collaborative, patient, and solutions-oriented mindset Role Requirements • Flexibility for occasional evening or weekend assessments • Valid driver’s license, car insurance, and ability to travel locally • Comfort balancing clinical oversight, team support, and relationship-building Responsibilities: • Conduct client assessments and develop personalized care plans. • Provide clinical oversight and ensure high-quality care. • Train, mentor, and support caregivers. • Collaborate with clients, families, physicians, and referral partners. • Promote a culture of compassion, accountability, and clinical excellence. Qualifications: • Active California RN license in good standing. • 10+ years of nursing experience in hospital, skilled nursing, home health, hospice, or related settings. • Strong clinical judgment and leadership experience. • Excellent communication, relationship-building, and mentoring skills. • Valid driver's license, reliable transportation, and ability to travel locally. Compensation: $120,000 – $150,000 yearly DOE + Bonus Opportunities
