Clinical Liaison – Home Health
Location: Fort Myers, FL
Compensation: Base Salary + Commission (Salary level impacts quota and commission start timeline)
Schedule: Monday–Friday (occasional evenings/weekends as needed)
Travel: Local travel within Lee County and surrounding areas
Overview:
We are seeking an experienced and relationship-driven Clinical Liaison to manage the referral-to-admission process for our home health services. The ideal candidate will have a strong Medicare home health book of business or, at minimum, established referral relationships with physicians, hospitals, skilled nursing facilities, and other key partners. This role combines business development with patient transition coordination to ensure a smooth, compliant, and positive experience for all parties involved.
Key Responsibilities:
Serve as the primary contact for hospital case managers, physicians, and other referral sources to facilitate patient transitions to home health.
Meet with patients and families to explain home health services, answer questions, and help develop a preliminary plan of care.
Maintain a minimum referral mix of 60% Medicare patients, with the remaining 40% from other sources (private duty, community resources, etc.).
Ensure patients meet CMS criteria for home health and that all required documentation supports a billable episode of care.
Build and maintain strong relationships with healthcare providers, discharge planners, and other key referral sources through regular visits and presentations.
Coordinate with internal teams to ensure timely and seamless admissions.
Conduct in-services and educational sessions for hospital and facility staff as needed.
Develop and execute marketing strategies to increase referrals and meet growth goals.
Collaborate with agency leadership to identify new business opportunities in the community.
Qualifications:
Minimum 2 years of experience in home health; transitional care or case management experience preferred.
LPN license required.
Existing Medicare referral sources strongly preferred.
Knowledge of Medicare guidelines and home health eligibility requirements.
Excellent communication, interpersonal, and presentation skills.
Self-motivated and goal-driven with strong organizational skills.
Proficient in Microsoft Office, including Outlook.
Valid driver’s license, reliable transportation, and current insurance.
Compensation Structure:
Base salary + commission.
Lower base salary options carry reduced quotas, allowing earlier commission eligibility.
Commission potential tied directly to meeting and exceeding referral targets.
