  • Responsibilities

    This position requires to be 75% on the road to visit clinics, all the clinics are in NYC.

    Our client, a physician services organization, is seeking a Clinical Liaison / MDS Program Representative to serve as the primary on-site liaison between physicians and nursing home staff, including MDS nursing teams, across a network of skilled nursing and long-term care facilities.

    This role serves as the owner’s on-site representative for physician services and MDS-related coordination, overseeing execution in the field, ensuring clear communication, and supporting effective clinical collaboration between physicians and facility teams. The position involves regular travel between approximately 15–20 facilities and serves as the central point of contact for day-to-day clinical and operational coordination.

    Responsibilities:

    Clinical & Operational Liaison

    • Serve as the primary communication link between physicians and nursing home staff, including MDS nurses
    • Ensure workflows, expectations, and clinical objectives are clearly understood and followed
    • Facilitate timely communication between physicians, nursing staff, and facility leadership

    Facility Oversight & Relationship Management

    • Travel regularly between skilled nursing and long-term care facilities to oversee day-to-day program execution
    • Build and maintain strong professional relationships with facility administrators, nursing leadership, and medical providers
    • Act as the go-to point person for questions, issues, and coordination related to physician services and MDS support

    Clinical Support & Follow-Up

    • Support physicians and nursing teams with scheduling coordination, documentation flow, and communication gaps
    • Identify operational challenges and escalate concerns appropriately, with proposed solutions
    • Monitor compliance with program guidelines and facility expectations

    Documentation & MDS Support

    • Assist with reviewing and coordinating clinical documentation as needed
    • Support teams with MDS-related coordination and communication (strong plus)
    • Help ensure accurate and timely documentation flow between physicians, nursing staff, and facilities

    Program Quality & Issue Resolution

    • Observe workflow effectiveness and recommend improvements
    • Address concerns proactively to maintain positive facility relationships
    • Ensure consistent program presence, professionalism, and service quality across all sites

    Qualifications

    Required

    • MDS experience is a MUST
    • Experience in healthcare coordination, clinical liaison work, provider relations, or nursing support
    • Strong interpersonal and communication skills
    • Ability to travel frequently between multiple facilities
    • Friendly, approachable, and professional demeanor
    • Highly organized with strong follow-through skills

    Preferred

    • Experience working with physicians and skilled nursing or long-term care facilities
    • Background in care coordination, utilization management, case management, or clinical operations
    • Comfort acting in an owner’s-representative-style role overseeing execution and relationships

    $150K
