    Qualicare Homecare is looking for compassionate, caring, and reliable individual to join our small yet feisty team as a Clinical Liaison. Qualicare location offers medical and non-medical home care services that focus on providing the best quality of life for our clients and peace of mind for their families.

    Job Summary The Clinical Liaison is responsible for establishing new referrers and educating referral sources, addressing challenges, building and maintaining relationships to secure referrals, completing Wellsky pre-admission screens, and contributing to the planning, organizing, and execution of the business strategic plan. Frequent day travel time local travel and occasional after-hours business development activities is required. The individual will be responsible for establishing and maintaining effective working relationships with referral accounts and facility leadership team members. All team members are responsible to respond to phone and email inquiries and commit to participate in establishing and implementing the highest standards of care.

    Responsibilities

    Drives new business activities with professionals and lay people

    Utilizes available technology to enable efficient processes

    Consults with physicians and other care providers related to client care services

    Consults with supervisors and staff regarding clients, families, and service programs

    Ensures compliance with standards of care

    Supports Director of Nursing in case of needed time away

    Acts as back up to conduct in-home assessments and creates care plans for clients

    Participates in quality assurance and organization risk management programs

    Qualifications

    Excellent customer service skills

    Self-motivated and enthusiastic go - getter who needs little prodding

    Current, in good standing, and active license to practice as a Registered Nurse.

    Previous clinical screening experience is preferred.

    Recent healthcare experience in a local rehabilitation, acute care, assisted living, or home health care agency required.

    Business development, marketing, and or sales expertise required

    Working experience of local medical community is preferred.

    Effective communication skills with medical, administrative, client, and family personnel, in an professional, assertive, yet warm manner.
