    We are seeking a Clinical Liaison who is passionate about connecting individuals and families with the care they need and supporting strong relationships with healthcare providers and community partners.

    Position Summary

    The Clinical Liaison serves as the primary connection between referral sources, clients, families, and our home care team. This role focuses on client intake, coordination of care plans, communication between healthcare partners, and ensuring high-quality, client-centered service delivery.

    Key Responsibilities

    Serve as the main point of contact for hospitals, clinics, physicians, and case managers to promote our non-skilled home care services.

    Conduct in-home or facility assessments to determine client needs and develop individualized care plans.

    Collaborate with caregivers and office staff to ensure seamless coordination of services.

    Educate referral sources and community partners about available home care options.

    Assist with follow up with clients and families to ensure satisfaction and adjust care plans as needed.

    Maintain accurate documentation and communicate updates in a timely manner.

    Participate in outreach events and community networking to strengthen referral relationships.

    Qualifications

    Current LPN or RN License

    Minimum 2 years of experience in home care, case management, hospital discharge planning, or related field.

    Strong communication, interpersonal, and relationship-building skills.

    Excellent organizational and documentation abilities.

    Compassionate, professional, and client-focused demeanor.

    Reliable transportation and valid driver’s license required.
