    Clinical Manager – Home Health (Full-Time, On-Site – Port Charlotte, FL)

    We are seeking a dedicated Clinical Manager (RN) to join our home health team in Port Charlotte, Florida. This is a full-time, on-site leadership role offering the opportunity to make a meaningful impact in patient care while ensuring compliance with state and federal guidelines.

    Position Overview

    The Clinical Manager is responsible for overseeing the delivery of high-quality home health services. This includes supervising clinical staff, coordinating patient care, ensuring adherence to care plans, and supporting the development of policies and procedures that drive excellence in care.

    Key Responsibilities

    • Provide leadership and supervision to nursing and support staff delivering patient care in the home health setting.

    • Coordinate patient care services, including assignments, referrals, and ongoing plan-of-care reviews.

    • Ensure compliance with all regulatory, licensure, certification, and accreditation standards.

    • Participate in the development and implementation of agency policies, procedures, and quality initiatives.

    • Evaluate staff performance, provide ongoing feedback, and promote professional development through training and mentoring.

    • Serve as a liaison with physicians, healthcare facilities, and referral sources.

    • Actively participate in case conferences, utilization reviews, and quality improvement activities.

    • Collaborate with leadership to plan and assess staffing needs, budgets, and community programs.

    • Support business development and community outreach efforts as needed.

    Qualifications

    • Active RN license in the state of Florida (required).

    • Minimum of two years of home health experience with at least one year in a supervisory or leadership role.

    • Strong clinical assessment, communication, and organizational skills.

    • Ability to manage multiple priorities in a fast-paced environment.

    • Bachelor’s degree in Nursing preferred.

    • Current CPR certification and Level II background clearance.

    Compensation & Benefits
