Position Summary
The Clinic Practice Management Nurse supports patient access, clinic flow, and operational efficiency at multiple primary care clinic sites Uptown on Magazine St.. This role primarily focuses on managing walk-in patients, supporting appointment scheduling, and coordinating real-time clinic flow in a high-volume primary care setting.
In addition, the nurse provides clinical triage and assists with in-clinic emergencies, including identifying patients who require escalation to a higher level of care such as transfer to the Emergency Department. The position works in close collaboration with the Chief Nursing Officer, Director of Nursing, providers, scheduling staff, and front desk teams to ensure safe, efficient, and patient-centered care.
Practice Management Systems Design & Performance Evaluation
Assist in the design, implementation, and maintenance of patient access workflows.
Collect, review, and interpret operational and access-related data to evaluate the effectiveness of scheduling, walk-in, and patient flow processes.
Coordinate, monitor, and evaluate scheduling workflows for specialty services, ensuring appropriate access, utilization, and alignment with clinic capacity.
Collaborate with scheduling staff, specialty clinics, and providers to address access barriers and optimize specialty appointment availability.
Monitor key practice management indicators related to access and productivity, including specialty scheduling performance.
Use data trends and operational observations to identify inefficiencies and recommend improvements.
Assist leadership in tracking progress toward clinic goals for patient access, productivity, and operational performance.
Support the ongoing maintenance and sustainability of access-related systems and workflows.
Clinical Triage & Emergency Support
Provide nursing assessment and clinical triage for patients presenting with urgent or concerning symptoms.
Assist providers and staff during in-clinic emergencies.
Identify patients requiring a higher level of care and assist with escalation to the Emergency Department, including:
Clinical assessment and stabilization as appropriate
Coordination with providers and leadership
Documentation and communication per clinic protocol
Serve as a calm, knowledgeable clinical presence during high-acuity situations.
Operational & Nursing Support
Assist the Director of Nursing with select operational and nursing tasks as delegated, including:
Supporting point-of-care testing controls
Emergency supply checks or restocking of supplies
Participate in daily morning huddles and contribute to discussions on staffing, access, and clinic flow.
Identify workflow challenges and recommend improvements to enhance patient access and clinic efficiency.
EPIC & Communication Support
Support EPIC workflows related to scheduling, access, and patient communication as assigned.
Communicate scheduling updates, delays, and access issues to providers and clinic staff.
Ensure accurate and timely documentation related to triage and patient flow decisions.
Required
Active, unrestricted RN or LPN license (per organizational policy)
Experience in an outpatient or primary care setting
Strong clinical triage skills
Ability to work effectively in a fast-paced, high-volume clinic
Proficiency with EPIC Electronic Health Record
Preferred
Experience working in walk-in or same-day access clinic models
Prior involvement in scheduling or practice management workflows
Strong understanding of patient flow and access optimization
