    About the Role:

    Join Golden Oaks Hospice, Inc. as a Clinical Supervisor and play a vital role in delivering enhanced hospice care outcomes! We are looking for a dynamic leader who is passionate about hospice care and dedicated to supporting our clinical team. The Clinical Supervisor is responsible for ensuring that patient care is coordinated and managed appropriately. The Clinical Supervisor oversees the care and services delivered by the clinical personnel.

    Golden Oaks Hospice, Inc. is a Joint Commission certified hospice agency located in the Chatsworth, CA.

    Responsibilities:

    Conduct regular assessments of patient needs and adjust care plans accordingly;

    Engage with families and patients to address concerns and enhance care experience;

    Initiate staff training and development programs to enhance team skills;

    Monitor clinical documentation for accuracy and completeness;

    Ensure compliance with all regulatory requirements and organizational policies;

    Provide mentorship and guidance to nursing staff, fostering a collaborative environment;

    Guide and instruct hospice personnel on accuracy of documentation;

    Provide field visits when necessary;

    Attends case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement;

    Assist agency with cost effective alternatives;

    Requirements:

    Active RN license in the state of California.

    Minimum of 3 years of nursing experience, preferably in hospice or palliative care.

    Previous supervisory or leadership experience in a clinical setting.

    Strong knowledge of hospice regulations and best practices.

    Excellent communication and interpersonal skills.

    Ability to work collaboratively with a diverse team of healthcare professionals.

    Compassionate demeanor with a commitment to patient-centered care.

    Proficient in electronic health records and clinical documentation.
