    Position Summary

    The Clinical Transition Nurse (CTN) plays a key role in the transition and admission of patients into hospice care. This position involves planning, assessing, and coordinating patient care in the home or residential setting. As a CTN, you will serve as both a clinical resource and a compassionate guide for patients and families navigating end-of-life care.

    Key Responsibilities

    • Conduct comprehensive assessments of patients and families during transitions into hospice services

    • Develop individualized care plans in coordination with the interdisciplinary team

    • Administer medications and perform prescribed nursing procedures

    • Provide patient and family education related to the hospice care journey

    • Supervise hospice aides and ensure quality and safety in delegated care

    • Serve as the clinical point of contact for families, referral sources, and internal teams

    • Maintain timely and accurate documentation that complies with regulatory and organizational standards

    Required Qualifications

    • Graduate of an accredited nursing program

    • Active RN license (California)

    • Current CPR certification

    • 1–2 years of recent acute care experience

    • 1–2 years of recent home care experience

    • Valid driver’s license and insured, operational vehicle

    • Physical ability to lift, position, and transfer patients and to perform prolonged walking/standing

    • Strong clinical assessment, planning, and documentation skills

    • Ability to work independently and collaboratively in the field

    Preferred Qualifications

    • Bachelor’s degree in Nursing (BSN)

    • At least 1 year of experience in hospice care

    • Exposure to holistic, complementary, or alternative care modalities
