    Position Summary

    We are seeking a compassionate and experienced Clinician (Social Worker or Nurse) to visit clients across various facilities and ensure their overall well-being. This role focuses on assessing patient needs, monitoring their physical and emotional health, and ensuring appropriate services and supports are in place. The ideal candidate is patient-centered, organized, and comfortable working independently while collaborating with interdisciplinary teams.

    Key Responsibilities

    Conduct regular on-site visits to patients in various facilities (such as nursing homes, assisted living facilities, community)

    Assess patients’ physical, emotional, and psychosocial well-being

    Ensure patients’ needs are being met, including medical, social, and supportive services

    Identify concerns or changes in condition and communicate findings to appropriate care teams

    Advocate for patients to ensure quality care and appropriate resources

    Coordinate with facility staff, families, and healthcare providers as needed

    Maintain accurate and timely documentation in accordance with organizational and regulatory standards

    Follow all confidentiality, safety, and compliance guidelines

    Qualifications

    Licensed Social Worker (LCSW, LMSW, or equivalent) or Licensed Nurse (RN or LPN)

    Current, unrestricted license in New York State

    Experience working with vulnerable populations, long-term care, behavioral health, or community-based settings preferred

    Strong assessment, communication, and interpersonal skills

    Ability to work independently and manage a flexible schedule

    Reliable transportation and willingness to travel between facilities

    Basic computer skills for documentation and reporting

    Preferred Skills

    Knowledge of care coordination and patient advocacy

    Experience with interdisciplinary teams

    Bilingual abilities a plus
