    Position Summary

    Focused on coordinating diabetes education, outreach and prevention activities including DPP instruction, coaching and follow-up. Focused on coordinating incentives, engagement, communication, and outreach activities for the overall population collaborative related to community partners and other key stakeholders.

    Primary Responsibilities include:

    • This is a position dedicated to our community-based diabetes program which coordinates efforts between clinical teams, coordinators, and sub-specialists for our DPP.
    • A solid understanding of diabetes and diabetes education is an essential component of this role.
    • Works with the providers and caregivers to coordinate medical and social services for the patient and sometimes for the care givers and uses standardized tools to access their needs
    • The health coordinator will use appropriate strategies, and methods to facilitate health promotion and disease prevention. Community outreach will be required.
    • Will supervise the Community Health Workers in their area and provide support as needed.
    • Will develop and provide educational programs may include, but not limited to, health screenings in the community specific to community healthcare needs.
    • Will monitor and ensure proper tracking, use and deployment of patient/participant incentives. Will work with community organizations, non-profits, and others to increase the types and availability of incentives to participants.

    Essential Duties

    • Conducts needs assessments, establishes priorities, designs, implements, and evaluates basic health education programs, protocols, and standards.
    • Coordinates a wide range of health education services, including establishing effective referral and publicity systems, monitoring quality and documentation, providing scheduling and logistical support, and facilitating use of community services to promote the delivery of cost-effective health education services.
    • Leads the development and deployment of direct group or one-on-one health education services to members and the public (virtually, telephonic, in-person, etc.).
    • Consults with physicians and staff regarding related health education services.
    • Coordinates health information projects such as program catalogs, newsletter, informational displays, and community health events
    • Work closely with community health center and specialty care staff to maintain communication and provide feedback and standardize procedures.
    • Identify new opportunities for cross-sector collaboration that support program implementation or address social determinants of health.
    • Perform other duties as assigned.

    Education and Experience

    • Associates degree in social work, counseling, behavioral health, psychology
    • At least 3+ years of related experience, or an equivalent combination of education, training, and experience.
    • Three or more years of experience working in health research, public health programs and/or community-based organizations that provide health services.
    • Experience with diabetes programs preferred and community-based outreach experience.
    • This is a position dedicated to our community-based diabetes program.
    • Experience in reporting, Word, Excel or google sheets a must.
    • The position requires a candidate to be competent in understanding of quality metrics, outcomes and dashboard development and management.
    • A solid understanding of diabetes and diabetes education is an essential component of this role.
    • Demonstrates leadership abilities
    • Able to handle multiple tasks at once, meet deadlines, detailed, flexible, able to manage relationships.
    • Excellent organizational and interpersonal skills.
    • Ability to work independently.
