Benefits offered for all full-time employees: • Medical • Dental • Vision • 401(k) • PTO • Life Insurance • Short and Long-Term Disability Responsibilities: • Assist patients in completing personal hygiene tasks like bathing, grooming, dressing, toileting, shaving, etc. • Mitigate issues with medications by helping to open bottles, organizing self-administered dosing, and arranging for refills, and help the patient with meal preparation and planning • Implement care plans as directed by the case manager, and keep updated and accurate care records • Ambulate patients as indicated in the care plan, and help patients complete exercises and physical therapy as needed • Complete basic medical tasks such as taking vital signs, measuring glucose and ketone levels, replacing bandages and dressings, and ensuring the proper use of medical devices • Administer Medications: Dispense medications as directed by the Registered Nurse (RN), maintaining an annual error rate of no more than one mistake • Resident Response: Attend to any and all resident call lights within 10 minutes to ensure responsive and attentive care • Light Housekeeping: Perform regular light housekeeping in resident rooms at least once weekly to promote a clean and comfortable living space • Keep each room up to high standards of sanitization by changing bedpans and sheets, and performing regular room cleaning • Service Charting: Accurately document all services provided without missing tasks, ensuring compliance with company standards • Assistance with ADLs: Support residents in Activities of Daily Living (ADLs) according to each individual’s Client Service Plan, with a zero-tolerance policy for missed tasks • Give patient support with basic care, daily living activities, bathing, getting dressed, going to the restroom, eating regular meals, and taking proper medication dosages • Activities: Encourage and initiate activities to enhance residents' mental and physical health and promote socialization. Qualifications: • 1-3 years of experience as an HHA/ home health aide, personal caregiver, nursing assistant, or personal care aid for a home health agency, at patient homes or care facilities settings is needed • High school diploma or GED is required - some college preferred • Possess a valid state driver’s license, current auto insurance, and reliable transportation • Extreme Ownership: Demonstrates accountability, ownership of outcomes, and proactively removes obstacles • Resourcefulness: Finds effective solutions even with limited resources and independently seeks knowledge • Passion for Purpose (Big Why): Possesses a strong, genuine dedication to serving and caring for vulnerable adults • Commitment to Growth: Actively pursues self-improvement and team betterment each day • Humility: Embraces a solutions-focused attitude, welcoming feedback, and putting egos aside for the best outcome • Reliability: Shows up on time, prepared, and ready to deliver dependable support • Gratitude: Maintains a positive outlook, seeking and appreciating the good in all situations • A compassionate, kind, and empathetic personality is essential, with superb organization and communication skills • Ability to work every other weekend • CBRF Certifications or willingness to obtain them during employment. • Current CNA license Compensation: $16 - $20 hourly
