    Position Summary:The Community Wellness Liaison (CWL) will perform community outreach in response to health care access needs in Southern Curry County. They will serve as a central resource for community questions and referrals for both consumers and providers. Build relationships with community resources, public health, schools, faith community and other service organizations to identify and develop resources and remove barriers to access health, housing and other social services.CORE JOB RESPONSIBILITIES:Essential Functions

    · Partner with Public Health Nurse to provide basic coordination of Public Health Services for Curry County

    · Provide reception services, registration of patients for Public Health Services, and general Outreach on medical services

    · Assist in testing and other Public Health tasks that are no required of a Licensed Provider

    · Coordinate with other agencies providing Public Health Service in Curry County

    · Complete Outreach events to educate the community on available services

    · Complete Community Health Worker training to become a CHW (cost and time covered by CCHC)

    · Establish and maintain the Coast Community Health Center resource list

    · Build community relationships and knowledge of services provided by Coast Community Health Center

    · Inform and assist homeless individuals with access to services and associated community resources; health, dental, mental health and basic needs.

    · Connect clients or consumers to information on publicly sponsored health insurance

    · Work with community stakeholders in educating the community on general programs and services

    · Maintain statistics/data on contacts; identify needs, problems and service gaps.

    · Attend appropriate community or networking meetings to facilitate outreach and gathering of information to support consumer access to healthcare.

    · Work with local schools, faith community, senior centers, law enforcement, community partners, and employers to build awareness of CCHC’s services.

    · Participate in community coalitions and related committees convened to prevent and reduce homelessness;

    · Serve as liaison between CCHC and the community

    · Attend school functions/meetings to educate faculty, parent/guardians and students about services offered

    · Gather information regarding health care needs of students, recommend/implement outreach strategies to increase student access to healthcare

    · Work to improve housing access and to assist in a planning grant to develop a more systemic program in Curry County

    · Attend Community Advisory Council meetings, health fairs and community events

    · Perform other related duties as assigned
