    POSITION SUMMARY:

    The Public Health Nurse will work to protect and improve the health Curry County residents through prevention, education and by providing a welcoming clinic environment for all people, including those from communities of color, teens, LGBTQ+ individuals and people with disabilities, and providing cultural responsiveness to needs and services. The Public Health Nurse will help provide services to the community including immunizations, communicable disease management, and reproductive health services.

    CORE JOB RESPONSIBILITIES:

    Vaccine Program Management

    · Provide vaccines to the community following program requirements and community needs, including administration or delegation to certified or other licensed staff

    · Maintain and apply OHA’s Immunization Protocols per program requirement

    · Run ALERT ISS on patients or review records for needs

    · Work with the school systems in Curry County on students facing exclusion, and vaccinate in order to keep children in school

    · Provide annual or other communicable disease vaccines as requested, and during formal vaccine clinics, including school events

    · Arrange for travel vaccines based on CDC recommendations and administer as available

    · Provide surge vaccine clinic capacity in the event of an outbreak of a vaccine preventable disease in Curry County.

    · Administer the vaccine program with awareness of funding sources and requirements per programs and insurance

    · Make sure staff has completed training yearly on immunization practices

    · Complete community education and outreach events in Curry County, including working with other medical providers to improve vaccination rates

    Reporting

    · Enter all vaccines in the Electronic Medical Record and ALERT IIS, including dose level eligibility codes

    · Order vaccines from the ALERT ISS system or other systems in exceptional cases, and complete inventory modules

    · Verify that at least two employees have current training and certification in vaccine storage, handling and administration, unless exempt under statute.

    · Report adverse vaccine reactions through the VAERS system

    Communicable Disease Management

    · Monitor and process electronic laboratory reports (ELR) for reportable communicable diseases in Curry County (excluding STI, HIV, and COVID-19)

    · Use all reasonable means to investigate in a timely manner all reports of Reportable Diseases, infections, or conditions. To identify possible sources of infection and to carry out appropriate control measures, Grantee shall investigate each report following procedures outlined in OHA’s Investigative Guidelines or other procedures approved by OHA. investigations.

    · Within Curry County, investigate the Outbreaks of Communicable Diseases, institute appropriate Communicable Disease control measures, and submit required information in a timely manner regarding the Outbreak to OHA in the Outbreaks database as prescribed

    · Conduct disease investigation and follow up for all vaccine-preventable diseases, excluding COVID-19, in accordance with OHA CD Investigative Guidelines, Oregon State Public Health Laboratory Protocols and Oregon Immunization Program Protocols.

    · Enter all case investigation information into Orpheus (State of Oregon reporting and tracking system) as appropriate and directed by OHA

    · Attend trainings as needed to fulfill the needs of the Communicable Disease program, including OHA Communicable Disease 101 and Communicable Disease 303 trainings

    · Provide Case Management services to all confirmed or suspected Hepatitis B Antigen-positive mother-infant pairs identified in Curry County, including education and treatment

    · Work with birthing hospitals serving Curry County on vaccination rates and seropositive pairs for tracking

    · Treat per Standing Orders or arrange for disease treatment with the Medical Officer

    · Provide COVID19 testing in Curry County as a low barrier service and report results to OHA

    · Provide education on COVID19 to those who are tested, and arrange follow up treatment as needed

    Reproductive Health Services

    · Provide education on family planning options in a culturally sensitive, and patient centered manner

    · Enroll patients for state service programs and explain the process for this program, as well as work within patient’s insurance parameters

    · Provide family planning materials, medications, and counseling on options for family planning that supports choice and decreases risk, after assessment of patient history

    · Arrange for Provider based services for family planning options that cannot be provided by this position, and follow the referral process until closed, as well as refer for Primary Care needs, as indicated

    · Manage supplies, ordering materials to be on hand for use

    · Arrange for interpreting services for all patients who need them

    · Be an ambassador for the Reproductive Health Program, providing outreach to the community with additional staff members

    · Serve as a Reproductive Health Coordinator (RHC) for program compliance and reporting to Oregon RH Program

    · Maintain privacy for patients and their care per state and federal law

    · Report all suspected or incidents of confirmed abuse as a State Mandated Reporter

    · Complete sexually transmitted infection screenings as indicated and/or requested

    · Arrange for breast cancer and cervical cancer screening

    OTHER JOB RESPONSIBILITIES:

    · Assisting the CCHC with clinical staffing needs as appropriate and needed.

    · Other job responsibilities as needed.

    QUALIFICATIONS (SKILLS, KNOWLEDGE & ABILITIES):

    Education, Training, Certification and Licensure:

    Minimum Qualifications/Transferable Skills*:

    • Possession of, or ability to obtain, an appropriate and valid license to practice as a Licensed Practical Nurse or Registered Nurse issued by the State of Oregon

    One of the following:

    • A Bachelor's degree in nursing from an accredited nursing program; OR

    • An associate degree in nursing from an accredited nursing program AND two years of experience

    • Possession of, or ability to obtain, an appropriate and valid driver's license

    • BLS CPR certification required

    • This position will include occasional shift coverage outdoors on evenings or on weekends

    • This is a field-based position that will be supporting mobile clinics and pop-up clinics for community events
