POSITION SUMMARY

You will be responsible for: 1) providing clinical expertise in caring for our clients and caregivers, and 2) helping our office with outreach activities with individuals within the home and long-term care industry.

REPORTS TO: Director of Operations

QUALIFICATIONS:

Previous management-level experience in the home, retirement and/or long-term care settings

Excellent interpersonal and outreach skills

Current license as a Registered Practical Nurse (Ontario)

Demonstrated organizational skills, flexibility, assertiveness, and team orientation

Demonstrated experience with MS Word and MS Excel

ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY

Leverage existing relationships and develop new ones with potential referral sources for clients and caregivers.

Showcase and deliver company’s clinical programs within the community.

Prepare a roadmap for outreach and community engagement.

Implement new clinical initiatives for clients and their families on behalf of the company.

Perform comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.

Develop an individualized plan of care for each client that incorporates clinical information and clinical best practices.

Review and deliver clinical training to staff. Over time, evaluate their performance and competencies.

Lead the implementation and communication of corporate clinical programs for the company.

Other activities as directed.
