What we're looking for:

Graduate of an accredited school.

Current LPN license in good standing.

Current CPR certification.

Reliable transportation.

Maintains confidentiality and HIPPA regulations.

Knowledge of safety and infection control practices.

What you will be doing:

Reporting changes for care plan updates, goals, outcomes.

Maintain client records showing assessments, interventions, evaluations.

Initiating referrals as needed to RN and careteam.

Basic patient care within scope of practice.
