ComForCare is seeking a RN Director of Clinical Services 20-30 hrs a week. This position offers a very flexible work schedule, with only one day a week in person, in the office during regular business hours. Duties would include:

Independently reviewing patient charts to identify potential needs for additional care.

Meeting weekly with our agency leadership to discuss and review the needs identified.

Coordinating follow up care with the patient's primary doctor's office if needed.

Performing new patient assessments and writing care plans as your schedule allows.

Updating existing patient care plans as needed.

Performing supervisory visits to our team of HHA's and CNA's as your schedule allows.

Providing training & assistance to our team of HHA's and CNA's as your schedule allows.

Providing leadership for PRN RNs doing infusion work, med box set up, wound care and the like.

This position is perfect for an RN that is looking for a part time job that offers flexible hours and a laid back work atmosphere, or a retired RN that still holds their RN license.

ComForCare Kent County is a growing Care Practice that works primarily with seniors by providing care and assistance within their homes. Our services include assistance with the activities of daily living including personal care, medication reminders, and light housekeeping. We also coordinate with medical professionals such as doctor's offices, hospitals, and skilled rehab centers and have a limited line of Private Duty Nursing patients receiving direct RN care.
