Will work under the Director of Nursing. The RN will be responsible for employee competency evaluations, employee orientation, employee in-services, employee supervision, and assessments. The RN will also be responsible for maintaining employee and client records to ensure they are kept up to date in compliance with regulations set forth by the NYS Department of health. Coordinate and supervise caregivers in the delivery of client care. Maintain compliance with agency policies, procedures and federal, state, and local regulations. Demonstrate proficient skills utilizing assessments for admission, transfer, reassessment, and discharged clients based on assessed needs.
Qualifications/Educational Requirements:
Registered Nurse (RN) with a current State of New York License and CPR certification
Minimum of two years of Home Care experience as an RN
Works collaboratively with clients, families, and staff
One year of Supervisory experience preferred
Responsibilities:
Complies with HIPAA, federal, state and local regulations
Perform yearly employee Competency evaluations on HHAs and PCA’s
Performs supervisory visits and monitors outcomes of delegation to ensure all direct care staff are adhering to the plan of care and care plan to meet the needs of the client
Provides employee inservice and orientation training
Demonstrates sound judgment and decision making skills throughout work assignments
Educate, coach and monitor employees on how to perform skills and allow employees to demonstrate skills competently for the RN Supports quality improvement initiatives
Will provide backup for the Director of Nursing in the following areas:
Performs initial client assessment, care plan and medical orders
Performs re-assessments as required by regulation, and if needed care plan updates, and medical order updates to physicians
Updates plan of care when there is a significant health status change in the client’s condition, change in service, physician orders or discharged from another facility
Initiates the plan of care under the physician or legally authorized practitioner orders as indicated to provide nursing services
Educate, coach and monitor client and family on disease process, medications, treatments and care plan to ensure client’s needs are met
Provides safe and effective nursing care and is an advocate of the client/patient/family
