JOB DESCRIPTION:
Community Access Individual Support are individualized supports intended to assist the person with the achievement, preservation, or improvement in skills. The services are offered to individuals who live in their own or family home. Personal care/assistance may be a component part of CLS services but may not comprise the entirety of the services. The amount of personal care/assistance is specific to the needs of the individual, as determined by the Supports Intensity Scale, the Health Risk Screening Tool, and other person-centered assessments. The service needs may be addressed in one or more of the following areas: Activities of daily living, personal grooming and health care, interpersonal relationships, mobility, home management, and use of leisure time.
Requirements:
HS Diploma or equivalent
Valid driver's license and reliable transportation;
CPR/First Aid certification
CPI Certification (training available)
Pre-Employment Negative TB test
7 Year MVR (Motor Vehicle Record)
Must be willing to complete Criminal background check
Must be able to utilize a computer/internet/Microsoft word
Must be able to complete mandatory online computer training courses
Responsibilities:
Social, leisure, and adaptive skill development that assists the individual in continuing home living.
Personal care, protective oversight and supervision as a component part but not as the entirety of the services.
Training in activities of daily living, such as bathing, dressing, toileting, transfers, hygiene, light housework, laundry, meal preparation, grocery shopping, using telephone, medication and money management
Medically related services, such as basic first aid, arranging, accompanying, providing individual transportation to medical appointments, and reminders and/or assistance self-administration of medication.
Transportation to grocery or personal shopping, banking, and other community activities supporting continued home living. Monitoring individual's condition by observing physical and mental state;
Recording individual's information by making entries in daily electronic system, notifying supervisor of changes or abnormal activity;
Implementation of a behavioral support plan to reduce inappropriate behaviors and to acquire new skills and behaviors.
Maintaining a safe, secure, and healthy environment by following agency and state policy and procedures.
Participating in routine training opportunities;
Ensuring that the agency remains in compliance by adhering to professional standards, home care and day program policies, procedures and federal, state, and local requirements.
Must be able to utilize computer/internet/Microsoft word to properly chart daily notes onto client databases.
