Nutrition Site Coordinator
General Summary
Responsible for programs and activities conducted at the assigned center(s). Maintains accurate data for meals served and activities conducted at center and keeps the center clean and safe for clients. The Nutrition Site Coordinator will also coordinate the transporting of meals to home-bound clients. This position will be on a Part-Time basis average between 25-29 hours a week.
Primary Responsibilities
1.Responsible to develop and submit a monthly activity plan for center participants to supervisor
2.Provides meal counts to kitchen or area office daily as directed by supervisor
3.Assures that the assigned center is maintained clean and safe and that city and state inspections are kept current
4.Collects, verifies and submits contributions and proceeds from center fundraisers to the accounting department at the central office
5.Coordinates transportation for clients, coordinates delivered meals as directed, and maintains vehicle records to document trips
6.Responsible to keep the center, dining area, storage area, floors, refrigerator, cooking appliances and other equipment clean and sanitized
7.Responsible for establishing collaborative relationships with community partners that may assist clients in meeting their family needs and goals.
8.Conducts home visits with clients to establish services and performs on-going assessments for home delivered meals.
9.Responsible for maintaining accurate and detailed records of all delivered and undelivered meals.
10.Ensures that facility, supplies, equipment and vehicles are kept clean, maintained in good working order, inventoried and in compliance with Federal, State and Local regulations and ordinances.
11.Ensures that all client intakes and the maintenance of documentation conforms with the regulations and standards related to Title 111 and the Title XX programs.
12.Any other duty as assigned by the supervisor
Work Experience
Must be able to obtain a health card and a food handler’s permit and pass a pre-employment physical and a criminal background check
Supervisory experience
Education/Certifications/Licensure
A high school diploma or the equivalent
Valid Texas driver’s license and a safe driving record
Associate’s Degree (Preferred)
Skills
Effective communication and people skills, excellent computer skills
Bilingual in English/Spanish
