Job Summary

We are looking for a Part Time Community Care Aide (Day Shift) to join our team! You will be directly working with patients, following a one-on-one care plan in patient's communities. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. Strong technology and verbal communication skills are needed to succeed in this role. Prior Home Health experience is mandatory. This shift can lead to full time if you are able to work in Philadelphia county with other clients.

Responsibilities

Assist patients with community activities such as going to museums, malls, shows, parks, etc . Must be able to work with wheel chair bound client, assist with feeding and toilieting.

Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor

Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert.

Engage patients in exercises or other activities. This position is a 2:1 (2 staff to 1 client).

Qualifications

High School Diploma or GED

Five year prior professional experience

Driver’s license required
