    Assisting Hands Home Care Cary Location: Cary & Raleigh, NC Position: Full-Time Schedule: Monday-Friday with one weekend per month.

    Assisting Hands Home Care Cary is looking for an experienced, compassionate, and highly organized Community Care Coordinator to join our growing team.

    This is an ideal position for a CNA or experienced home care professional who has hands-on caregiving experience and is ready to take the next step into a coordination and leadership role.

    The Community Care Coordinator will oversee care delivery across multiple senior living communities in Cary and Raleigh, ensuring residents receive reliable, compassionate, and personalized care while supporting caregivers, families, and our office team.

    Our Community Care program is different from traditional one-on-one home care. Caregivers may assist multiple residents throughout the day, making strong organization, communication, time management, and leadership skills essential to success.

    What You'll Do

    Visit and regularly check in at multiple senior living communities in Cary and Raleigh

    Check in with caregivers and residents to ensure scheduled services are being completed

    Follow up with clients and families to ensure they are satisfied with the care they are receiving

    Communicate clearly and professionally with residents, families, caregivers, community staff, and the Assisting Hands office

    Identify and address concerns quickly and professionally

    Handle challenging situations involving clients, caregivers, and family members with patience, professionalism, and confidence

    Provide ongoing training, coaching, and support to caregivers

    Train new caregivers on Community Care procedures, expectations, and client-specific tasks

    Ensure caregivers understand and follow detailed care schedules and task lists

    Monitor caregiver performance and provide feedback when needed

    Communicate changes in client needs or care plans to the office

    Help coordinate caregiver schedules and coverage when needed

    Follow up on missed tasks, schedule changes, and client concerns

    Ensure medication reminder protocols and other Community Care procedures are being followed

    Maintain strong relationships with senior living community staff and leadership

    Keep the Assisting Hands office informed of what is happening in each community

    Attend weekly office meetings and provide updates on each location

    Assist with improving systems and processes as our Community Care program continues to grow

    What We're Looking For

    The ideal candidate is someone who:

    Is a CNA or has significant hands-on home care experience

    Understands the daily challenges of working with older adults and their families

    Has experience providing personal care and assisting with activities of daily living

    Is comfortable working with clients who may have dementia, confusion, behavioral challenges, or changing care needs

    Can confidently handle difficult conversations and challenging situations

    Is comfortable addressing concerns with caregivers and holding team members accountable

    Communicates clearly, professionally, and consistently

    Is highly organized and able to manage multiple locations and priorities

    Follows through and does not let issues fall through the cracks

    Has strong problem-solving skills and can think independently

    Is comfortable training and coaching caregivers

    Can build positive relationships with clients, families, caregivers, and senior living community staff

    Understands the importance of documenting and communicating changes promptly

    Is reliable, dependable, and able to work independently while remaining connected to the office team

    Has a valid driver's license and reliable transportation to travel between Cary and Raleigh locations

    "Are our clients receiving the care they need?" "Are our caregivers supported and following the care plans?" "Are families happy with the communication and care?" "What needs to be addressed before it becomes a bigger problem?"

    You will play an important role in making sure our clients receive the quality of care they deserve and that our caregivers feel supported and prepared to provide it.
