The Community Health Navigator provides culturally responsive, harm-reduction-oriented outreach, navigation, and care coordination services to individuals who use drugs, with a focus on Black, Indigenous, and People of Color (BIPOC) communities disproportionately impacted by substance use, overdose, homelessness, and structural inequities. This role builds trusted relationships, connects participants to health and social services, and supports improved health outcomes through non-judgmental, participant-centered engagement.
REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
2 years of experience working with underserved communities in direct service capacity
Direct experience or experience working with people experiencing homelessness and/or experiencing behavioral health or substance use issues
Passionate about providing direct service
Understanding of social determinants of health
Strong knowledge of behavioral health conditions and barriers to accessing care
Strong de-escalation skills
Proficiency with MS Office – Word, Excel, Outlook
Ability to develop and maintain relationships with staff and partner organizations
Excellent oral and written communication skills, and ability to connect with a diverse audience
Ability to work independently with limited supervision
Demonstrates cultural competency
Ability to work a flexible schedule including nights and weekends
DESIRED KNOWLEDGE, SKILLS, AND ABILITIES:
Navigated current behavioral health systems through work or personal experience
Advanced knowledge of the social determinants of health that affect Black, Indigenous, and People of Color
Understanding of upstream and downstream factors
Direct/lived experience or experience working with people experiencing homelessness, behavioral health issues, and/or substance use disorder
