Avail Health & Behavioral Solutions is seeking a motivated and compassionate Community Health Worker (CHW) to support individuals and families in navigating healthcare systems, accessing community resources, and improving overall health outcomes.

The CHW will serve as a bridge between clients, providers, and community services to reduce barriers to care and promote self-sufficiency.

This is a field-based role requiring strong communication, organization, and advocacy skills. Key Responsibilities

Conduct outreach and engage individuals in the community

Perform needs assessments and develop action plans

Assist clients with accessing medical, behavioral health, housing, food, and transportation resources

Support care coordination with physicians, case managers, and service providers

Provide education on chronic disease management, medication adherence, and preventive health

Assist clients with scheduling appointments and completing applications

Maintain accurate documentation and case notes

Participate in interdisciplinary team meetings

Track progress toward client goals and outcomes

Preferred Skills

Experience working with underserved populations

Knowledge of housing navigation, employment resources, or re-entry support

Bilingual a plus

Experience with electronic documentation systems

Flexible work from home options available.
