A medical based company is looking to fill a 2 month remote contract Community Health Worker position.

Pay rate: $24/hr (Paid weekly)

Hours: 10AM-6PM

Allmed Benefits: Dental Insurance, Health Insurance, Vision Insurance and 401(k)

Contract: 08/04/2025 to 10/31/2025

Job description w/ details:

You will be working with the Quality & Enrollee Team for KY Health Plan, 20 people total on team.

You will complete “access to care” assessments over the phone with KY Medicaid members and enter the responses into Community Care that will close the gap in care for SNS-E.

Required Qualifications:

KY Medicaid experience

Strong communication skills, empathy, active listening, motivational interviewing, cultural competency, clear documentation, technology proficiency, organized

Perfered Qualifications:

Healthcare background, CHW experience, healthcare experience, KY Medicaid experience, experience with SDOH needs
