We are seeking a community liaison person to connect persons with disabilities with community resources. Ideal candidate must reside or familiar with Rhode Island neighborhoods. This position requires outreach to different communities, contacts with Medicaid recipients; providing support, and direction to community resources. Some office duties are required. Bi-lingual speakers are encouraged to apply.

Qualifications:

Certified Community Health Worker by RICB

Ability to communicate well in English, and other languages, preferably Spanish

Previous case management experience is a plus

Ability to prioritize and multitask

Excellent written and verbal communication skills

Strong attention to detail

​Strong organizational skills

Proficient in the use of computer
