    Shelter Services Community Health Worker (CHW) Serves to support the Care Team, shelters and clinic by providing outreach to the Southeast Neighborhood and the medical outreach program in Southeast. Emphasis on transportation, coordinating daily routes from shelter services to community partners and engaging unsheltered individuals across Southeast Roanoke by providing mobile support, follow up care coordination and warm hand offs to other agencies. Requires collaboration within the Rescue Mission, and with community partners.

    This individual will not be a driver for the organization.

    Reports to: Director of Shelter Services

    Hours of Availability: Monday – Friday; hours may vary; evenings as scheduled

    Location: Rescue Mission of Roanoke, VA and surrounding community

    Compensation: $21.63 (non-exempt/hourly)

    · Demonstrated attention to detail/strong organizational skills to assist with multiple daily assignments and special projects.

    Job Specific Qualifications:

    · Working knowledge of Microsoft Office and EMRs

    · Working knowledge of local community resources

    · Ability to relate to individuals in crisis as well as local organizations and agencies

    · Community Health Worker certification – obtain within 6 months

    Career Summary (not all-inclusive list):

    The CHW advocates to remove barriers and ensure equality for the unsheltered and homeless, conduct field outreach (community canvassing/education events), assist patients/guests with Medicaid and Advance Directive enrollment, DMV, Social Services and other timely documentation, data collection and reporting. Track transportation utilization, outreach contacts, service connections, and outcomes. Reporting data metrics to appropriate partners, Rescue Mission and United Way.

    Lead coordination of a daily transportation route connecting emergency shelter with local day shelters and local service providers. Ensure consistent access to daytime services. Engage unsheltered individuals across Southeast Roanoke, not limited to those already seeking services at the Rescue Mission. Provide mobile support, follow-up care coordination, and warm hand-offs to other agencies. Serve as a liaison between service providers to reduce duplication, close gaps in care, and strengthen referrals and case coordination.

    All team members may be asked to complete other duties as assigned by the leadership/management with an expectation to complete these assignments as requested.
