    Position Overview

    CLFC Healthcare & Communications is recruiting Community Health Workers to deliver care navigation and family support for the Bexar County System of Care in San Antonio, Texas. The System of Care connects children, youth, and young adults who have complex behavioral health needs, along with their families, to coordinated treatment and community resources through a single family-centered process.

    This role fits a certified Community Health Worker who is energized by direct, relationship-driven work in the community: meeting families where they are, removing the barriers that keep them from care, and walking alongside them through the behavioral health system. CLFC carries the credentialing, payroll, liability, and reporting infrastructure so the Community Health Worker can focus on families.

    Key Responsibilities

    • Coordinate intake and assessment for children, youth, young adults, and families referred through the System of Care.
    • Navigate families to behavioral health treatment, primary care, and community resources, and coordinate referrals across providers.
    • Provide ongoing case management support, including benefits and Medicaid enrollment assistance and follow-up on service connections.
    • Engage and retain families through consistent outreach, home and community visits, and culturally responsive communication.
    • Coordinate with behavioral health providers, schools, and community partners on each family's plan.
    • Participate in Community Resource Coordination Group (CRCG) meetings and System of Care staffings.
    • Complete accurate documentation and data collection required by Bexar County and SAMHSA.
    • Conduct outreach and engagement activities that build awareness of the System of Care in the community.

    Required Qualifications

    • Texas Department of State Health Services (DSHS) certified Community Health Worker, or eligible to obtain certification within the first 90 days of hire.
    • High school diploma or equivalent.
    • Experience in community health, social services, behavioral health navigation, or a closely related field.
    • Strong interpersonal and communication skills with diverse children, youth, and families.
    • Valid Texas driver's license, reliable transportation, and willingness to travel for community field visits across Bexar County.
    • Ability to pass a criminal background investigation and program onboarding screening.

    Strongly Preferred Qualifications

    • Bilingual English and Spanish.
    • Experience serving children, youth, and families with serious mental health or behavioral health needs.
    • Familiarity with System of Care or wraparound service models.
    • Experience with Medicaid and public benefits enrollment.
    • Lived experience in or strong ties to the communities served in Bexar County.

    Compensation and Engagement Details

    • $41,800 to $50,600 annual salary as a full-time W-2 employee, commensurate with experience and certification.
