    Job Description

    Community Healthcare Network is seeking for a Full-Time Health Education/ Family Planning who will implement a wide range of clinical and community health education activities, with a focus on promoting sexual and reproductive health and related health services. Work as part of a team to provide coordinated patient care (medical, educational, and social services). 

    Please note, the position is REMOTE for the time being with possibilities of reporting to Health Clinics in Future

    DUTIES AND RESPONSIBILITIES:

    • Designs, implements and evaluates the clinic's overall patient education program.
    • Conduct clinic educational sessions-group and individual-to allow for informed consent of patients.
    • Conducts group and individual sessions on reproductive health topics (reproduction, contraceptive methods, family planning, pregnancy, relationships, HIV, STDs, etc.)
    • Conceptualizes and creates lesson plans on reproductive health for both youth and adults.
    • Markets CHN’s reproductive and sexual health workshops to NYC schools and community-based organizations.
    • Facilitates reproductive health curriculum at NYC schools, CBOs and other venues throughout NYC.
    • Organizes, reviews and suggests educational materials for ordering and distribution; maintains an inventory of all clinic educational materials.
    • Completes necessary reports and forms, and documents patient services in the appropriate medical record.
    • Identifies community resources for health education resources and/or services.
    • Assists in the development, distribution and implementation of educational and clinic promotional materials.
    • Attends and participates in training/informational sessions that are appropriate for Health Educators and in all clinic staff meetings.
    • Attends community meetings as a representative of the clinic and/or agency as assigned by Director of Health Education.
    • Plans, schedules and conducts health education and outreach presentations for community groups.
    • Plans and participates in special CHN events.
    • Facilitates related services for health center patients and CHN staff as appropriate, with respect to their confidentiality and privacy.
    • Other duties as assigned.

    KEY INFORMATION: 

    • Job Type: 35 Hours per week (Full-time).
    • Compensation: $47,516K per year.

  • Qualifications

    Qualifications

    CERTIFICATE/LICENSE REQUIRED:

    • B.A. or B.S. in Community Health Education or related health discipline.
    • At least one year’s experience working in a community health setting that required contact with patients/clients.
    • Bilingual/Spanish (Preferred).
