Community Healthcare Network, is a Federally Qualified Health Center (FQHC) serving the underserved communities of Brooklyn, Bronx, Manhattan and Queens. Through the care we provide at our Health Centers and through our various programs, we touch the lives of 85,000 people annually.

We are seeking for a Full-Time HIV Nutrition Program Manager who will oversee the HIV Nutrition Program. The Manager will work directly with a diverse HIV-positive patient population and closely with HIV Program staff and the Director of Nutrition, to coordinate clinical services. The HIV Nutrition Manager will also plan and facilitate Nutrition related events, activities, and workshops.

JOB FUNCTION:  

RESPONSIBILITIES INCLUDE, BUT ARE NOT LIMITED TO: 

• Evaluates and monitors patients’ nutritional status of patients living with HIV/AIDS.
• Conducts nutrition education and counseling sessions.
• Plans and facilitates nutrition workshops for the agency.
• Organizes and develops nutritional care plans and individualized therapeutic diets as needed.
• Completes necessary reports and forms and documents patient services in the appropriate medical record.
• Facilitates related services for health center clients as appropriate with respect to their confidentiality and privacy.
• Identifies nutrition related patient education materials for the use by the agency.
• Conferences with the Director of Nutrition as necessary to seek consultation and share information that is pertinent to patient care.
• Performs other related duties, as assigned.

KEY INFORMATION: 

• Job Type: 35 Hours per week (Full-time).
• Compensation: Ranges between 60,000K - 65,000K per year.
• Supportive integrated team environment.

WHAT WE LOOK FOR: 

• RD/RDE preferred with New York State Certification.
• Minimum of B.S. in Nutrition or Health Education with experience. Masters preferred.
• BILINGUAL SPANISH STRONGLY PREFERRED.
