Home Health / Hospice Community Liaison
Join a fast-growing Midwest provider helping seniors and families thrive through high-quality, holistic home health and hospice care. You'll have local control, strong clinical support, and leadership that clears roadblocks instead of creating them.
Build and grow referral relationships across hospitals, SNFs, ALFs, LTCs, and clinics
Maintain consistent territory coverage to keep referrals flowing
Educate community partners on care programs and admission criteria
Coordinate with clinical teams for smooth, fast starts of care
Track referral activity and admissions vs. set goals
1+ year in home health or hospice sales/marketing
Valid driver's license, insurance, and reliable vehicle
Clear background and drug screen
Ability to travel locally (≤1 overnight/quarter)
Comfortable managing goals and field activity metrics
Pay: $75K base + incentive plan
Benefits: Health, dental, vision, PTO
Mileage reimbursement: $0.70/mile
Backing of a stable parent company with start-up agility
Mon–Fri, 8am–5pm
Onsite + daily field travel
No on-call
