A Day After Day Home Care Agency is seeking an outgoing, professional, and relationship-focused Community Liaison to help build referral partnerships and expand our community presence.

Part-Time: Approximately 10–15 hours/week

Location: Indianapolis & surrounding areas

Schedule: Variable based on business needs

Mileage: Approved business-related mileage reimbursed according to company policy

Growth: Opportunity for increased hours and responsibilities as the company grows

Responsibilities

Build relationships with hospitals, nursing/rehab facilities, senior living communities, hospice, and healthcare professionals.

Conduct community visits, networking, and outreach.

Promote our home care and transportation services.

Follow up with referral partners and develop new referral opportunities.

Track outreach activities and referrals.

Qualifications

Strong communication and networking skills

Outgoing, professional, and self-motivated

Comfortable with phone calls and face-to-face meetings

Healthcare, home care, sales, or community outreach experience preferred

Growth opportunities available as the company expands.
