Position Summary: The Community Liaison plays a key role in representing ComForCare within the local community. This position focuses on building and maintaining relationships with referral sources such as hospitals, physician offices, senior centers, rehabilitation facilities, hospice and community organizations. Unlike a traditional marketing role, this position is specifically tailored to support the mission of home care—connecting clients and families to the care they need.
Key Responsibilities:
Develop and nurture relationships with local healthcare providers, community organizations, and referral sources.
Represent the company at community events, health fairs, and networking functions.
Educate the public and potential partners about our services and the benefits of home care.
Coordinate with the internal team to ensure a smooth referral and intake process.
Track outreach activities and report on community engagement and referral outcomes.
Identify new opportunities for partnership and service expansion.
Qualifications:
Proven experience in community outreach, healthcare marketing, or a related field.
Strong interpersonal and communication skills.
Knowledge of the home care or healthcare industry is preferred.
Self-motivated, organized, and comfortable working independently.
Valid driver’s license and reliable transportation for travel within the community.
Ability to pass a background check and drug screen.
