Job Title: Community Living Support Worker – Direct Care Staff
Reports To: Program Supervisor / Manager
Job Summary:
The Community Living Support Worker provides direct care and support to individuals with developmental disabilities, mental health conditions, or other support needs to help them live as independently as possible in the community. This role promotes personal growth, daily living skills, social integration, and health and safety in accordance with individualized support plans.
Key Responsibilities:
Provide hands-on support with daily living activities (e.g., hygiene, dressing, meals, medication reminders).
Support individuals in achieving goals outlined in their Individual Support Plans (ISPs).
Facilitate community involvement by accompanying individuals to appointments, recreational activities, shopping, and social outings.
Encourage and assist with skill development in areas such as cooking, cleaning, budgeting, and transportation.
Maintain a safe, clean, and supportive living environment.
Monitor and document client progress, behaviors, and incidents according to agency policies.
Communicate effectively with team members, families, and professionals involved in care.
Adhere to all policies, procedures, and ethical standards, including confidentiality and dignity of clients.
Participate in ongoing training and team meetings.
Qualifications:
High school diploma or GED required; additional training or education in human services preferred.
Previous experience working with individuals with disabilities or mental health conditions is a plus.
Valid driver’s license and reliable transportation (may be required for community outings).
Ability to pass background checks and health screenings.
Strong interpersonal, problem-solving, and communication skills.
Compassionate, dependable, and patient.
Working Conditions:
May include evenings, weekends, and holidays.
Work is performed in home and community settings.
Some lifting, bending, and physical assistance may be required.
