    Job Description

    POSITION SUMMARY:

    The Health Educator is responsible for supporting the Team Care model by providing educational services to patients.  Education tasks and weekly assignments will be coordinated by Health Education Manager based on CMC goals. The Health Educator reports to the Health Education Manager, with Center Manager daily oversight.

     

    MINIMUM REQUIREMENTS:

    *          BA degree and two (2) years of experience working in health education, or five (5) years of experience working in related field

    *          Bilingual in English and Spanish; written and verbal

    *          Valid California driver's license, proof of insurance, and personal transportation

     

    SPECIFIC DUTIES:

    *          Promote exceptional patient experience through adherence to CMC team-based care practices

    *          Participate in and attend team huddles

    *          Assists in patient quality assurance activities, including patient outreach efforts, data entry, and reporting

    *          Acts as site advocate by marketing Health Education Department and its services, pursuing and taking warm hand offs, tabling in lobby, and doing presentations to staff

    *          Refers patients to other Care Team members for items outside of their scope

    *          Communicates regularly with Care Team to ensure best health outcomes for the patients

    *          Conducts group and individual education sessions to patients on health related topics within the department guidelines

    *          Share information with patients using patient centered techniques like motivational interviewing

    *          Completes appointment documentation in the EHR per Health Education workflow guidelines

    *          Makes follow-up appointments as needed

    *          Attends and participates in staff meetings and trainings

    *          Conducts presentations in assigned service area for outreach

    *          Under the supervision and approval of the Health Education Manager, assists with monthly health campaigns

    *          Assists in the development of educational materials and curricula through the correct approval process

    *          Respond to Well App messages related to health outreach campaigns

    *          Performs other duties as assigned

    *          Travels to different sites as assigned

     

    PERFORMANCE REQUIREMENTS

     

    KNOWLEDGE, SKILLS AND ABILITIES

                *          Knowledge of educational programs and materials

                *          Knowledge of routine office practices

                *          Knowledge of computer systems and applications

    *          Knowledge of grammar, spelling, and punctuation to compile data and type reports

    *          Knowledge of adult learning and patient centered education and counseling techniques

                *          Ability to review and edit reports and correspondence

    *          Ability to identify problems, provide information, and recommend solutions

    *          Ability for public speaking and group facilitation

    *          Ability to conduct individual and group educational classes & conduct presentations

    *          Ability to communicate clearly including information on sensitive issues including sexual activities and drug-related practices

    *          Ability to prepare routine records and reports using terminology common to program

    *          Ability to interpret, adapt, and apply appropriate written guidelines and work practices

    *          Skill in establishing and maintaining effective working relationships with employees and public

    *          Skill in being communicative and able to comfortably and openly share health topic information

               

     

     

    TYPICAL PHYSICAL DEMANDS:

    Work may require sitting for long periods of time; also stooping, bending and stretching for files and supplies.  Occasionally lifts boxes and materials weighing up to 25 pounds.  Requires manual dexterity sufficient to operate a keyboard, calculator, telephone, copier, and such other office equip­ment as necessary. Vision must be correctable to 20/40 and hearing must be in the normal range for telephone contacts.

     

    TYPICAL WORKING CONDITIONS:

    Exposure to communicable diseases, medicinal preparations and other conditions common to a clinic environment.
