Position Summary
The Community Outreach & Family Support Coordinator is responsible for educating, engaging, and supporting pregnant and postpartum mothers, and families in CT, with a focus on Medicaid awareness, eligibility, and enrollment. This position leads community-based outreach efforts, facilitates educational workshops, builds partnerships with healthcare and social service providers, and provides hands-on assistance to families navigating and not limited to completing Medicaid applications.
Key Responsibilities:
Educational Workshops & Seminars
Plan, organize, and facilitate educational workshops and seminars regarding the maternal health and medical home programs
Adapt presentations and materials to meet the needs of diverse audiences and community settings
Community Outreach & Engagement
Conduct outreach activities in collaboration with healthcare providers, maternity clinics, community centers, other local partners, and other FAVOR staff
Distribute culturally appropriate informational materials, brochures, and flyers related to Medicaid eligibility and application processes
Represent the organization at community events, health fairs, and partner meetings
Identify gaps in access to Medicaid information and share insights with program leadership
Medicaid Application Assistance
Provide direct assistance to pregnant and postpartum mothers and other families with completing Medicaid applications, including in-person and guided online support
Explain required documentation and help families prepare needed materials
Language Access & Equity
Ensure Medicaid information and outreach materials are available in multiple languages
Coordinate or provide interpretation and translation support during workshops, outreach events, and application assistance
Community Partnerships
Establish and maintain partnerships with community organizations, women’s shelters, healthcare providers, and community clinics
Serve as a liaison between families, partner organizations, and internal program staff
Follow-Up & Ongoing Family Support
Implement follow-up systems to track application progress and ensure mothers complete the Medicaid enrollment process
Provide continued support to address application issues, denials, or documentation requests
Maintain accurate records of outreach activities and metrics, applications assisted, and follow-up outcomes
Program Support & Reporting
Assist with grant reporting and documentation as required
Participate in team meetings, training, and continuous improvement efforts
Required Qualifications
Bachelor’s degree in social work, public health, human services, community health, or a related field (or equivalent experience of 2 years in a related position such as working as a social service worker)
Experience in community outreach, health education, case management, or benefits enrollment support
Knowledge of Medicaid programs or public benefits systems (or willingness to learn)
Strong communication and interpersonal skills
Ability to work independently and manage multiple outreach activities
Preferred Qualifications
Experience working with pregnant or postpartum populations
Bilingual or multilingual skills
Familiarity with healthcare, maternal health, or social service systems
Experience providing application or enrollment assistance
