Position Summary
Wrapped N Love Home Care is seeking a Community Outreach & Intake Specialist to drive referral growth, strengthen community partnerships, and oversee the intake and on-boarding process for new clients.
This is a field-based, boots-to-ground role combined with structured intake coordination. The ideal candidate is relationship-driven, performance-oriented, and comfortable working in a KPI-focused environment where results, responsiveness, and execution matter.
This role bridges community outreach and operational on-boarding — ensuring that referrals convert efficiently into active starts of care while maintaining a smooth and professional client experience.
Key Responsibilities
Community Outreach & Referral Development • Build and maintain strong relationships with referral sources including hospitals, facilities, social workers, case managers, and community partners. • Conduct in-person outreach visits to expand agency visibility and referral opportunities. • Develop and grow new referral partnerships (“power partners”) within assigned territories. • Maintain consistent follow-up and engagement with existing referral sources.
Referral Management & Conversion • Serve as the primary point of contact for incoming referrals. • Respond quickly to new referral inquiries and initiate outreach to families or representatives. • Guide prospective clients through service options and on-boarding steps. • Manage follow-up cadence to improve referral-to-start-of-care conversion rates.
Intake & Client On-boarding • Conduct intake assessments and on-boarding visits (including Medicaid and transfer clients). • Ensure all intake documentation is complete, accurate, and compliant. • Coordinate collection of required authorizations, consents, and service agreements. • Prepare completed on-boarding files for hand off to the care coordination team.
KPI & Performance Execution • Track and manage performance metrics related to response times, conversion rates, intake completion, referral source growth, and contribution to billable hour growth. • Maintain accurate real-time tracking and documentation within company systems. • Participate in weekly KPI review discussions and performance evaluations.
Performance Expectations
Success in this role is measured by: • Referral responsiveness and follow-up discipline • Referral-to-start-of-care conversion performance • Intake completion volume and accuracy • Growth of active referral sources and community partnerships • Contribution to billable hour growth • Tracking accuracy and operational reliability
Ideal Candidate Profile
• Experience in healthcare outreach, intake, admissions, sales, or community relationship building preferred • Strong communication and relationship-building skills • Highly organized with strong follow-up habits • Comfortable working independently in both field and office environments • Performance-driven mindset with ability to work toward measurable goals • Strong problem-solving skills and professional judgment • Ability to balance speed, accuracy, and empathy during intake conversations
Work Environment
• Combination of community-based outreach and office-based intake coordination • Local travel required for facility visits, meetings, and on-boarding appointments • Collaborative, growth-focused environment emphasizing accountability and results
Compensation & Incentives
• Hourly compensation based on experience • Eligibility for discretionary performance recognition bonuses tied to KPI performance
