    Community Partnership for Families of San Joaquin

    The Court and Career Navigator will work to ensure CPFSJ has the capacity to meet client needs by building relationships with the Superior Courts, business community, and local service provider network.  The navigator will work independently and use their professional judgement to meet assignments. The primary focus of this position is to manage relationships between CPF and the broader community where clients work and live. This is to ensure that institutional barriers are reduced in efforts to increase client’s ability to navigate difficult social and legal challenges.

    RESPONSIBILITIES:

    • Initiate contact, establish rapport, and communicate effectively with clients, staff, community members, courts, and local business
    • Cultivate and leverage relationships with the court to assist clients and staff navigate courts/institutional processes
    • Cultivate and leverage relationships with the business community to assist clients and staff navigate local economic opportunities.
    • Research and build capacity for prosocial activities
    • Develop a team of informal service providers (neighbors, church leadership, and others) and formal providers to support clients in developing and implementing their case plan/service goals
    • Conduct home visits and/or meet with clients and families at diverse and remote sites. including the San Joaquin County Jail, Superior Courts, schools, and libraries
    • Maintain accurate case files as directed in a timely manner and according to professional standards and funder’s timeline
    • Participate in data collection and other evaluation processes and procedures as necessary
    • Enter in data from files into database and check for accuracy
    • Complete case notes on a regular basis and ensure that all notes are mirrored or typed up into the database
    • Participates in service integration related training, including cross-training
    • Participate as part of a service team, interacting with Service Integration Coordinators, Career and Family Navigators, Youth Services Coordinators, and other community support members as needed
    • Performs case management and administrative functions as an integral member of an interagency integrated service team. Willing to learn Integration concept in delivery of services
    • Works with informal service providers (neighbors, church leadership, and others) to ensure they have appropriate support as they work with children and families in the context of integrated service plans
    • Assists with FRC or other community-based family support platform general operations to ensure success of integrated service process. 
    • Participates in job development training and service integration related training, including cross-training
    • Develops and implements a plan to recruit program participants
    • Develops and implements a plan and strategies for participant retention
    • Conducts outreach activities to engage new referral sources and participants
    • Works with SJC Probation officers and Human Service Agency (HSA) to recruit and engage participants.  Performs intake and assessment services for prospective program participants; determines readiness for job training programs
    • Develops individual service plans based on participant input, interests, and needs
    • Maintains participant records and completes required documentation/reporting, including any requests of supervisor, funders, and/or program evaluators
    • Coordinates participants’ participation in programs with collaborating agencies
    • Networks with other providers/organizations to generate community resources for participants
    • Assists participants with researching and accessing needed services including additional job training, educational support, counseling, housing, childcare, medical services and others
    • Coordinates service provision with referring agencies
    • Develops and performs mentoring, coaching and follow up strategies to ensure retention of participants
    • Conducts, facilitates, and organizes workshops under the direction of the E&T Supervisor  
    • Prepares and provides monthly and yearly reports about E&T programs and opportunities 
    • Other duties and may supervise others as assigned

     EDUCATION/EXPERIENCE:

    • Bachelor’s Level with at least one year experience preferred in Human Services, Criminal Justice, Vocational Rehabilitation, Counseling, Social Work or relevant area preferred. Associate’s degree or some college considered with considerable related experience.
    • Thorough understanding of challenges and barriers faced by formerly incarcerated individuals struggling to succeed in the community after release (psychological, logistical, financial, legal, stigmatization, family-related, peer-related, etc.)
    • Ability to work with incarcerated and previously incarcerated individuals from multiple ethnic backgrounds.
    • Ability to conduct home visits and/or meet with participants at diverse and remote sites.
    • Ability to scan the area and avoid potential dangers when entering risky environments.
    • Ability to learn the neighborhood-based Family Resource Center (FRC) approach to providing social connections, positive peer associations, and community supports.
    • Ability to learn and incorporate the principles of family support and protective factors.
    • Knowledge of community resources within local area.
    • Involvement with grassroots organizations, resident organizations, and neighborhood groups.
    • Ability to work with probation officers and personnel from other service providers, including developing case plans involving multiple organizations.
    • Strong personal organization and time management skills; Working under pressure.
    • Ability to see possibilities and strengths, not obstacles or deficits.
    • Ability to facilitate multi-dimensional team meetings.
    • Ability to work cooperatively with the system “as is”, maintaining positive partner and system relations—while at the same time advocating for change in a collaborative, constructive and positive manner.
    • Basic proficiency (or the ability to learn) basic word processing (e.g. MS Word).
    • Demonstrated communication skills.
    • Ability to pass and maintain a background clearance as required for access to secured facilities, school sites, and other organizations serving youth.
    • Must adhere to all CPFSJ policies, partner agreements (MOUs, LOAs, Contracts, etc..), federal, state, and local laws and regulations.
    • Must maintain Youth Mental Health First Aid Certificate.

    REQUIREMENTS

    • Must be authorized to work in United States. 
    • Must provide own transportation and have a valid driver’s license and provide proof of insurance.
    • Pass a background check.

    ABILITIES AND PHYSICAL REQUIREMENTS

    • Sit for extended periods of time.Stand and walk for a long period of time, bend over, reach overhead, grasp, push, pull and move, lift and/or carry at least 30 pounds to waist height. Enter data into a computer terminal/typewriter, operate standard office equipment, and use a telephone
    • See and read a computer screen and printed matter with or without vision aids
    • Hear and understand speed at normal levels and on the telephone.
    • Employees in this position will be required to work indoors in a standard office environment, and come in to direct contact with CPF’s staff, other agencies and the public.
    • Employees in this position will also be required to outreach to target neighborhoods and schools, and work outside of the office atmosphere.

    COMPENSATION:

    Starting Pay Scale: Competitive Wage, Based on Proven Experience.  Full-time, non-exempt position subject to overtime and comprehensive benefits including medical, dental, vision, life and retirement.  

    Monday through Friday, 8:00 a.m. to 5:00 p.m.
