Compass Healthcare Consulting & Placement is conducting a search for an experienced Enrollment Specialist for CDPAP Program. The Enrollment Specialist CDPAP position is for a Licensed Home Care Agency located in Long Island City, Queens, NY. Qualified candidate will have prior Home Health Care, CDPAP Coordination or Enrollment experience, with a Licensed Home Health Care Agency. HHA Exchange experience, CDPAP, CDPAS, Manager, Supervisor, Specialist, Lead Coordinator, Homecare Home health

About the Position: Enrollment Specialist CDPAP will oversee the CDPAP enrollment process including on-boarding of new patients and CDPAP care givers who provide services and activities within the CDPAP Program. This position will be able to work 2-3 days in office and the other 2-3 days will work from home.

Enrollment Specialist CDPAP Program will be responsible for:

• Oversees the enrollment and on-boarding process for CDPAP Program patients and care givers
• Make virtual visits with patients and care givers, walking them through the application process
• Ensures all paperwork is returned completed and signed
• Ensures all proper documentation is obtained from CDPAP care giver, for on-boarding process
• Ensures newly enrolled patients receives care and services they qualify for, in a timely manner

Required Skills

• Minimum of 2 Years of previous experience in a Home Care setting
• CDPAP experience, required
• Knowledge and  previous use of HHA Exchange
• Patient oriented
• Solid time management and problem solving skills
• Great interpersonal skills
• Excellent communication skills (written and verbal)
• Strong attention to detail
• Highly organized
• Able to multitask efficiently and effectively
