The Home Health Licensed Vocational Nurse provides direct patient care under the supervision of the Registered Nurse. Working collaboratively with team members, the Licensed Vocational Nurse follows the plan of care to effectively meet outcome goals of patient care and improve their overall well-being. The Home Health Licensed Vocational Nurse regularly communicates with the Registered Nurse overseeing the patient to ensure quality care management.

Requirements include: -1 year of experience as a Licensed Vocational Nurse in the home health capacity -Current LVN License -Current CPR Card (BLS for Healthcare Providers) -Current TB (within 1 year) -Current Physical (within the last 6 months) -Current Driver’s License -Current valid auto insurance

Bilingual skills are a plus as we serve many Spanish speaking communities

Experience and certification to draw blood, stage wounds, change catheters is a plus.
