    We are currently seeking to fill the Hospice Medical Social Worker position.

    -Position to be performed at Fresno Office, 93711, and remotely, throughout our service territories .

    -Medical office background required.

    -Compensation (negotiable, based on experience, etc.).

    -Opportunity to accumulate overtime.

    About the Position

    The Hospice medical social worker is responsible for the implementation of standards of care for medical social

    work services and as defined by the state Medical Social Work Practice Act.

    Qualifications

    We’re seeking a flexible, self-motivated individual with:

    Graduate of a bachelor’s program in social work accredited by the Council on Social Work Education. May also have a master’s or doctoral degree in social work.

    Minimum of one (1) year's experience in health care.

    Experience in hospice care preferred.

    Demonstrates excellent verbal and written communication, and organization skills.

    Possesses and maintains current CPR certification.

    Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.

    Awareness of hospice and the social aspects of sickness, loss and mortality, as well as knowledge of chronically sick people.

    Capacity to utilize knowledge of the special requirements of hospice patients and family members.

    Essential Functions:

    Assesses the psychosocial status of patients related to the patient's illness and environment and communicates findings to the registered nurse.

    Carries out social evaluations and plans intervention based on evaluation findings.

    Maintains clinical records on all patients referred to social work.

    Prepares clinical and progress notes.

    Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs.

    Provides information to parents or families/caregivers and community agencies.

    Serves as liaison between parents or families/caregivers and community agencies.

    Maintains collaborative relationships with organization personnel to support patient care.

    Maintains and develops contracts with public and private agencies as resources for patient and organization personnel.

    Participates in the development of the total plan of care and case conferences as required.

    Assists the physician and other team members in understanding significant social and emotional factors related to health problems.

    Participates in discharge planning.

    Other dues as delegated by the Clinical Director/Supervisor.

    Advise patients and their relatives in making health care decisions following their objectives.

    Contact local services to aid the patient and their families with final requests.

    Aid with funeral preparation.

    Identify a patient’s religious and psychological needs and provide appropriate assistance if needed.

    Assist the patient's family with the necessary arrangement and paperwork after death occurs.

    Identify the other needs of the patient and support their circle and assist as their need.

    Education/Experience Required:
