    JOB SUMMARY

    In accordance with agency Mission, Vision, Values and Goals, the Community Support Specialist assists eligible individuals in gaining access to needed treatment and rehabilitation, as well as other medical, social, and educational services and supports; arranges and coordinates assessments of the individual's need for treatment and rehabilitation; coordinates and monitors service and support activities. The Community Support Specialist functions as the primary contact for individuals served, as well as for other agencies, within the framework of a team model.

    PRIMARY FUNCTIONS

    Provides community based support, case management services, and group education to assigned population as defined by department. Assists assigned clients in gaining access to resources, both internal and external, including, but not limited to, rehabilitative, medical, social, educational, and psychiatric.

    Completes all documentation as clinically outlined and in a timely fashion as defined by departmental standards. At a minimum: progress notes are to be completed within the electronic medical record within 48 hours and all treatment plans and reviews have measurable goals and objectives.

    Arranges, coordinates and assists supervisor in completing the annual assessments and treatment plans for assigned clients within the outlined timeframe.

    Serves as a vital multi-disciplinary treatment team member, and facilitates program, departmental and agency communication/operations through required attendance at treatment team meetings, departmental meetings, supervisory meetings, and other meetings as assigned.

    Participates in and meets ethical/agency standards regarding all treatment and documentation practices, QA/QI procedures, utilization management, emergency procedures, and program/client outcomes.

    Responds to crisis needs of assigned clients during office hours and provides appropriate intervention. Responds to client need immediately 100% of the time. Serves as back up for other CSS’ in client crisis situations as requested.

    Follows all agency policies and procedures including emergency procedures.

    SECONDARY FUNCTIONS

    Provides case management and crisis intervention for non-assigned team clients as requested by supervisor

    Provides Consultation and Education for the community as assigned.

    Serves on departmental, program, or agency committee(s) or task team(s) as assigned.

    QUALIFICATIONS:

    Education - Bachelor's degree in psychology, social work, nursing, counseling or closely related human services field. If the degree is in a field other than human services, a bachelor's degree and four years of experience in mental health is required.

    Experience - Equivalent experience may be substituted on the basis of one (1) year of experience for each year of required educational training.

    Licenses, Certifications, or Registrations - None required.

    REQUIREMENTS FOR POSITION:

    Must become Privileged Staff according to Medicaid guidelines to provide services for consumers if required by the department. Must acquire and maintain training related to job performance including, but not necessarily limited to: Agency Orientation, Program Orientation, CPR training (every two years), First Aid training (every two years), Non-Violent Crisis Intervention (every two years), Infection Control, Medication Training, Cultural Competence (6 hours every two years), and Documentation Training.

    Must complete eighteen (18) hours per year with documented attendance of professional development/training or education related to case management, community specialist to the special needs of the population within assigned department.

    Must maintain a valid drivers license. Must have access to dependable auto with adequate auto insurance and registration, for making frequent home/community visits.

    Available to work weekends and evenings if required by department.

    DEGREE OF SUPERVISION:

    Community Support Specialist must receive supervision by the Team Leader or Clinical Supervisor and additional education/guidance as necessary. Treatment Reviews need to be signed by Team Leader or Clinical Supervisor, as defined by the department.
