    JOB SUMMARY

    In accordance with agency Mission, Vision, Values and Goals, provides clinical assessments and referrals to client/families that require the initial clinical assessment to enter into services. The Intake Clinician conducts the intake and initial orientation process in its entirety to ensure assessment completeness. Coordinates with nursing, counselor and/or community support staff to complete intake process.

    Regular hours will be Monday through Friday 12 pm to 8 pm. Training hours will be 8 am to 4:30 pm until clinic opens. Must be flexible on hours to check on clinic on Saturdays.

    PRIMARY FUNCTIONS

    Completes clinical assessment upon intake of new or returning clients to include bio-psychosocial assessment with complete diagnostic assessment, assignment of clinician in Anasazi, as well as any other assessments as defined by department. All documents must be filled out completely and accurately, and timely.

    Assigns clients, and schedules appointments if appropriate, to a primary clinician and/or CSS most appropriate to support the client’s needs or requests.

    Completes referral forms to other departments, if applicable, and follows up as needed until client is linked with primary service provider.

    Enters assessment into appropriate EMR, follows written protocol and submits all paperwork as defined by departmental procedures.

    Completes assigned case tasks as assigned (ie: therapy, discharge summaries, follow up calls to clients, providing resources to clients, etc.)

    Provides crisis management as needed to client/family during initial visit or provides appropriate resources as needed.

    Participates in and meets ethical/agency standards regarding all treatment and documentation practices, QA/QI procedures, utilization management, emergency procedures, and program/client outcomes.

    Follows all agency policies and procedures including emergency and corporate compliance policies/procedures 100% of the time.

    SECONDARY FUNCTIONS

    Attends and participates in weekly treatment team meetings.

    Provides crisis management services to client/families as needed during initial visit.

    QUALIFICATIONS

    Education:

    Master’s degree preferred from an accredited college or university in social work, psychology, counseling or closely related field.

    Experience:

    Preferably 1 year experience in the provision of mental health/substance abuse treatment services.

    Licenses, Certifications, or Registrations:

    LCSW or LPC – preferred, licensed, certified, recognized or registered

    Computer skills

    REQUIREMENTS FOR POSITION:

    This is a full-time (40 hrs/week) position.

    Cardio-Pulmonary Resuscitation: Renewal every 2 years

    First Aid: Renewal every 2 years

    Cultural Competency: All agency staff are required to gain 3 hours annually of staff development training in the area of Cultural Competency.

    Training which meets yearly standards of 36 hours every two years of training related to substance abuse/treatment, 10 hours of which meet be outside training.

    Ethics – 3 hours annually

    DEGREE OF SUPERVISION:

    This position reports to the Program Director, who provides weekly supervision.
